Managing Your Diabetes
Being overweight is the number one cause of Diabetes. The “Diabetes Prevention Program (DPP)” is a program focused onhow to lose weight in a healthy and constant way.
You can lose 5 to 7% of your weight in the first phase of 16 sessions of 2 hours each and an annual follow-up
Includes personal monitoring by a health promotor, free A1C tests, blood pressure, consulting and referral to clinics
We work with you to prevent Type-2 Diabetes. We start with a 16-week group-based class to help participants:
- Education and how to mangae your diabetes
- Nutrition classes and cooking tips (Cooking Matters)
- All of Us
- First Aid Certification
- Physical Activity FREE classes for all levels
- Strengthen Families
- Youth Mental Health First Aid
- CONFIANZA (Confidence)
- A1C (a common blood test used to diagnose type 1 and type 2 diabetes) FREE test
Cohorts start at different times during the year. We highly recommend potential participants call us.
